Typically, the pain experienced by patients who are "bone-on-bone" originates from the surrounding structures. These structures overcompensate to support the joint due to the lack of cartilage. Pain may also be associated with swelling in the bone and tears in other structures like the meniscus or ligaments surrounding the knee, which may also be treatable.

The goal of the procedure is not necessarily to regrow cartilage, as this is not always achievable, but to treat the knee comprehensively, including the cartilage and other structures that contribute to knee pain as well. This approach aims to improve joint function, reduce pain, and potentially delay or avoid surgery.

Invasive knee surgery can take months of rehabilitation, aggressive pain management strategies, and risks that may be avoidable.
